Information for Providers

The North Carolina Healthy Transitions Initiative has developed a non-clinical support program for young people in Western North Carolina. Participants of the Healthy Transitions program will be able to opt into transition facilitation, educational/vocational support, and/or peer support services.

To be eligible for services, interested participants must meet the following criteria:

  • Be ages 16-25 years old
  • Reside in Buncombe or Henderson Counties
  • Have a current DSM-5 mental health diagnosis

How is the Healthy Transitions program tailored to the specific needs of transition-aged youth and young adults?

  1. We are a non-clinical program that allows participants to receive support from us along with their medical, clinical, and therapeutic services. Healthy Transitions can operate as either a complementary or a stand-alone service, depending on the needs of the individual.
  2. We are a low-barrier support service that does not exclude interested young folks from enrolling in our program based on finances, insurance coverage, or location within Buncombe or Henderson counties.
    • We are grant-funded, so no insurance is required
    • Our community-based staff works with participants wherever they feel most comfortable
    • We can provide Comprehensive Clinical Assessments (CCAs) in the community or virtually
    • We provide case management to connect youth and young adults to the resources they need
  3. Youth and young adults have a choice in the services they want to receive and can opt-out of any portion of the program at any time
  4. Healthy Transitions is youth and young adult-driven, meaning that the program is rooted in what young people are saying that they need from a support service like ours. To learn more about youth and young adult-driven programming, please visit our young adult advocacy page.
