Connect, Support, Care

North Carolina Healthy Transitions, funded by the Substance Abuse Mental Health Services Administration (SAMHSA), is a program that helps young people (ages 16-25) dealing with mental health challenges get the support they need as they move into adulthood. Growing up and figuring out adulthood can be tough, and getting the right support shouldn’t be confusing or stressful. That’s where we come in!  

  • We are currently working in Iredell County to make sure young people can get the support they need, without getting lost between child and adult services. Our goal is simple: to connect you with the right resources and to help you feel supported, so you don’t have to figure out everything on your own. 

What we Offer

  • Outreach – We connect with young people and the community to raise awareness about mental health and make it easier to find support. 
  • Assessments – We help you figure out what kind of support would work best for you and then help to get you connected with the right services. 
  • Care Coordination – We make it easier for you to access services, whether that’s counseling, housing, or other resources to reach your goals. 
  • Peer Support – We offer support from people with lived experience, who can better understand what you’re going through and can help you navigate services. 
  • School & Job Support – We support you in reaching your goals, whether that means finding/keeping a job, making a resume, going back to school, or learning a trade.
