North Carolina Healthy Transitions is an initiative funded by the Substance Abuse Mental Health Service Administration (SAMHSA) aimed at implementing a coordinated system of outreach, assessment, and care coordination for transition-aged youth. The overall goal of Healthy Transitions is to improve access to treatment and support services for youth and young adults ages 16 – 25 with a serious mental health condition and improve their functioning as they transition into adulthood.

  • Henderson and Buncombe counties are the local communities that will implement comprehensive awareness, engagement, infrastructure, policy, and service delivery approaches specific to this age group that is seamless across child and adult serving systems.

Healthy Transitions Services:

  • Outreach to young people and community stakeholders to improve service access and public awareness of the needs of young people with serious mental health challenges
  • Clinical Comprehensive Assessments to determine a young person’s specific needs and direct them to the appropriate service
  • Care Coordination to assist young people in accessing services and supports based on their own individual goals
  • Peer Support that promotes young people’s active participation in treatment and helps young people navigate services
  • Education and Employment Support to assist young people with education and employment goals
